Bilateral traumatic hip dislocations are extremely rare. Most of these are related to acetabular or proximal femoral fractures. most have attributed the cause to high-velocity motor crashes. Bilateral traumatic hip dislocations are less common (only 0.01%–0.02% ) of all dislocations affecting both hips in which one hip dislocates towards the posterior side while the other hip is on the anterior side. The initial diagnosis and early management are very to prevent patients from complications and adverse outcomes. The initial manifestation involves a reduction in incidence and development of avascular necrosis before other surgical procedures. Advanced traumatic life support (ATLS) is considered as first-line support for hip dislocation patients while a physical examination is a must for examining the direction and extent of dislocation for further management. The clinical appearance in the case of posterior dislocation involves an internally rotated, foreshortened, and fixed leg which is reversed in the case of anterior dislocation in which the leg is externally rotated while in the case of the hip reduction process, a neurovascular examination and assessment are also done A case is reported of asymmetrical bilateral traumatic hip dislocation with an associated fracture of the acetabulum and femoral head occurring in a young adult with no previous history of hips abnormality.
