Popis: | Widening of the re t ropharyngeal space as a result of nasopharyngea l tumors, laryngeal tumors, esophageal tumors and abscesses is a condit ion well documented . 1,2,6, ~ T h e widening has also been associated wi th cervical ver tebral fractures. I n recent years, hemorrhage into the re t ropharyngeal space af ter carot id a r te r iography and carotid surgery has been recognized2 '4 Only recently has this complicat ion been no ted as a cause of death. 4 We have es t imated t h a t 1 or ~ per cent of hematomas tha t occur following puncture of the carotid ar tery reach sufficient size to threaten the pa t ien t ' s airway. I n order to de tec t this potent ia l occlusion of the airway, every pat ient undergoing ar te r iography is examined for hemorrhage in this area. F r equen t l y there is no evidence of hemorrhage anter ior to the sternocleidomastoid muscle, or in the pre t rachea l area. The pa t ient may no t have a pharyngea l dysarthria , but f requent ly a change in the voice can be detected which should alert one to the possibi l i ty of hemorrhage. When a potent ia l ly serious hemorrhage is present, a bulge, which m a y be bluish, can often be seen in the oropharynx. T h e absence of hemorrhage anterior to the s ternocleidomastoid muscle m a y give a false sense of securi ty, as pressure m a y be developing which will occlude the pharynx wi thout present ing anter ior ly . The pharyngeal space should be inspected by opening the mou th and observing the pa la te and the posterior pharyngeal wall as low as possible. If this is found bulging forward i t is a serious sign. |
