Complication of laparoscopic tubal banding procedure: Case report

Popis: Laparoscopic tubal sterilization involves placing a small silicone ring over a segment of the Fallopian tube avoiding the injuries associated with methods using cautery. A case history is presented to illustrate a possible complication associated with laparoscopic tubal sterilization. A 30-year-old woman (gravida 3 para 2 abortus 1) underwent laparoscopic bilateral tubal banding with the Falope Ring. The only abnormality noted during the procedure was a 3 X 3 cm cyst on the right ovary which was aspirated. 3 weeks later the patient was readmitted due to a 7-day history of lower abdominal colicky pain. The symptoms worsened and a presumptive diagnosis of intermittent ovarian torsion was made. The operation revealed a granulomatous reaction in the segments containing the rings. No generalized peritonitis or other adhesions were noted. A right salpingo-oophorectomy and left partial salpingectomy were performed. Again the postoperative course was uncomplicated. In another study of 2643 Falope Ring sterilizations tubal transection and postoperative pain were the 2 most common complications. It is thought that the case persented illustrates hypersensitivity reaction of the peritoneum to the silicone rings. 2 photomicrographs show the left Fallopian tube granulomatous reaction and the acute inflammation to the tube near the ring on the right Fallopian tube. Although this presents only 1 case history the diagnosis of hypersensitivity reaction should be considered in patients with persistent abdominal pain and a history of tubal banding sterilization.
