Popis: | A 32-year-old man with a history of recurrent sinusitis, bronchitis, and tonsillitis since 12 years of age came to our clinic. He reported diarrhoea of more than 2 weeks’ duration. The patient had been diagnosed with common variable immunodefi ciency (CVID) 6 years before and had been treated since with monthly immunoglobins. Because of the patient’s recent history of diarrhoea, gastrointestinal transit, ileoscopy, and ileal biopsy were done. The gastrointestinal transit (fi gure) showed accelerated transit and several nodular repletion defects in terminal ileum, compatible with nodular follicular hyperplasia. Ileoscopy confi rmed lymphoid hyperplasia. Among enteropathies associated with CVID, moderate increase in intestinal intraepithelial lymphocytes occurring in CVID can be the cause of chronic diarrhoea in these patients. Diff erential diagnoses include lymphoproliferative diseases and infl ammatory bowel disease. |
