Popis: | The historical perspective of the use of visceral replacement after resection of the esophagus for cancer is discussed and reasons for the preferred use of colon are given. Whatever technique is used, it is accepted that, at the present, without very strict selection and preparation of patients for surgery an operative mortality rate of about 20% will occur. Details of operation are given in a series of 26 patients undergoing radical resection for cancer of the laryngopharynx and upper esophagus with repair by isoperistaltic colon transplant through a retrosternal passage. The results of treatment are briefly discussed. It is emphasized that 70% of the cases reported in the article underwent "salvage surgery" after the failure of previous full-dosage irradiation. A plea is made for combined treatment by a planned combination of modified dosage irradiation and radical surgery. |
