The Influence of Oral Contraceptive Therapy on the Periodontium—Duration of Drug Therapy

Popis: | The research objective was to investigate the interrelationship between the responses of the periodontal tissues to plaque in subjects who had been taking oral contraceptives (OCs) for various periods of time compared with control subjects. Study participants were females between the ages of 18-40 years who were attending the Margaret Pyke Center in London. The subjects were divided into 3 groups: those who had used OCs regularly but for less than 5 years; those who had used OCs regularly for more than 5 years; and a control group who used an alternative form of contraception. For inclusion in the control group it was stipulated that the subjects must have either never used hormonal contraceptives or alternatively have terminated a course of therapy of less than 1 years duration more than 12 months previously. A total of 151 women were selected for the study. 49 had been taking OCs for over 5 years 63 for less than 5 years and 39 formed the control group. The Plaque Index of Silness and Loe was used to assess plaque; the modified Gingival Index was used to measure gingivitis. Depths of pockets were measured with a Williams probe to the nearest millimeter. There was an increase in the Gingival Index with duration of therapy particularly in the younger age group. The combined analysis of variance for both age groups indicated that this result was signficant at the 0.05 level. There was signficantly more inflammation in the older subjects for all the periods of medication. There was no signficant change in level of attachment related to the period of medication but there was significantly greater loss of attachment in the older age group. The Gingival Index values were related to the level of plaque and duration of therapy. In general there was increased inflammation with increased periods of medication. This finding was signficant at the 0.05 level of significance. There was signficantly more inflammation as plaque scores increased for all 3 groups of subjects. The level of attachement is related to the plaque scores and duration of therapy. The analysis of variance for the combined data relating plaque scores and duration of therapy to loss of attachment was not significant. There was an increase in loss of attachement with age which was significant at the 0.01 level. |
