A 51-YEAR-OLD MALE was operated on in October 2004 with bisegmentectomy (segments II and III) and cholecystectomy for hepatocellular carcinoma. Past medical history included hepatitis B-related liver cirrhosis, esophageal varices status post endoscopic band ligation, and duodenal ulcer. Over a 3year follow-up period, his serum alpha-fetoprotein (a-FP) concentration has been repeatedly measured and a progressive increase from 10.59 to 11.61 ng/ ml was noted since April 2007. Physical examination revealed a progressively enlarging, irregular margin mass in the umbilicus. The mass measured approximately 2 cm in diameter and the surface was ulcerated with sero-sanguineous discharge (Fig 1). A chest x-ray was normal. An abdominal computed tomography scan showed a 3.4 3 2.0-cm soft tissue lesion in the umbilical region, as well as a 1.4-cm hypodense lesion in the lower segment V of the liver (Fig 2). The umbilical mass was widely excised with the full thickness of the abdominal wall. The 3.8 3 2-cm mass penetrated through the umbilicus deeply into the peritoneal cavity (Fig 3).
