Activation of the EEG by Procyclidine Hydrochloride in Temporal Lobe Epilepsy

Popis: | SUMMARY The ability of procyclidine to provoke subclinical seizure activity in the EEG when administered intravenously has been investigated in a series of 63 patients. Using a dosage of 10 mg the EEG's of 32 out of 40 patients considered to have temporal lobe epilepsy were activated by the criteria as defined. No activation was, however, produced in a series of 23 non-epileptic patients who were suffering from a variety of neurological and psychiatric disorders and who had normal resting EEG's. In one patient with organic brain damage pre-existing EEG abnormalities were enhanced. A dosage of 20 mg of procyclidine was found to be significantly more effective than 10 mg. No EEG activation was obtained in a series of 19 “control” patients when the larger dose of procyclidine was used. Six patients considered to have temporal lobe epilepsy had EEG recordings with sphenoidal electrodes under thiopentone narcosis with negative results. In 3 of these subclinical seizure activity was observed when 20 mg of procyclidine was injected intravenously after the patients had been allowed to emerge from the thiopentone narcosis. No serious side-effects were observed in association with the use of procyclidine in doses of either 10 mg or 20 mg. Nevertheless, as with all anti-cholinergic agents, caution should be exercised in its administration to elderly subjects and in those liable to either glaucoma or urinary retention. It is concluded that the intravenous administration of procyclidine hydrochloride may be a useful procedure in the investigation of any patient considered to be suffering from temporal lobe epilepsy, and that it may be preferable to the use of hyoscine by displaying relatively fewer side-effects. RESUME Les auteurs etudient chez 63 patients la possibilite de provoquer des activites electro-encephalographiques epileptiques infracliniques par injection intraveineuse de procyclidine. Avec des doses de 10 mg les EEG de 32 patients sur 40 qui etaient consideres souffrir d'une epilepsie du lobe temporal ont ete actives selon les criteres definis d'avance. Aucune activation ne fut observee chez 23 patients non epileptiques qui souffraient d'affections neurologiques ou psychiatriques diverses et qui presentment un EEG de repos normal. Chez un patient qui etait porteur d'une atteinte cerebrale organique des anomalies electroencephalographiques preexistantes ont ete renforcees. Une dose de 20 mg s'est montree d'une facon significative beaucoup plus efficace que la dose de 10 mg chez les epileptiques, mais ne procura cependant aucune activation chez 19 sujets de controle. Trois parmi six patients supposes souffrir d'une epilepsie du lobe temporal et qui ne montraient pas d'activites electroencephalographiques specifiques malgre un enregistrement par electrodes sphenoidales au cours d'une narcose avec thiopentone, ont presente de telles activites lorsqu'ils furent soumis a l'injection de 20 mg de procyclidine intraveineuse a la fin de leur narcose. Aucun effet secondaire important n'a ete provoque par l'injection intraveineuse de 10 mg et meme 20 mg de procyclidine. Cela n'empeche que, comme avec tous les agents anti-cholinergiques, des precautions doivent etre prises lors d'une administration chez des sujets âges ou susceptibles de presenter un glaucome ou une retention urinaire. Il est conclu que l'administration intraveineuse d'hydrochlorate de procyclidine peut representer un procede utile pour l'etude de tout patient susceptible de souffrir d'une epilepsie du lobe temporal. L'utilisation de ce produit semble preferable a l'usage de l'hyoscine dans la mesure ou elle procure des effets secondaires moins importants. |
