Popis: | A VICIOUS circle often exists in patients with extensive bilateral "universal" bronchiectasis. The greater the loss of lung substance, the more vital that airways be free. As the number of bronchiectatic segments is increased, the functionable segments are reduced. This means that when the ventilatory mechanism can least afford obstruction of airways, there are larger collecting reservoirs from which the bronchi are flooded. Therefore, one of the primary aims in management, when it is not feasible to perform excisional surgery, has been to promote adequate bronchial drainage.
