Popis: | 1.1. The records from 150 patients who have undergone resection for abdominal aortic aneurysm have been reviewed. Fifty of these patients were operated upon during the early period of this study and declamping of the aorta was carried on using a variety of methods. Nearly all of these patients experienced declamping shock. Five patients (10 per cent) died during declamping as the result of ventricular fibrillation or cardiac arrest. 2.2. The declamping procedure was changed to include the use of a leak-proof woven Teflon prosthesis and the administration of a vasopressor and buffering agent into the prosthesis prior to declamping. The later period of the study includes one hundred patients in whom the procedure was used. There was no incidence of declamping shock nor were there any deaths during declamping. 3.3. In eighteen patients pH, pO 2 , and pCO 2 were serially monitored during declamping. These studies indicated that sudden metabolic acidosis occurred in these patients even when buffers were used prior to declamping. |
