Carotid reconstruction for Takayasu's (pulseless) disease using open thromboendarterectomy at aortocarotid junction

Popis: | Safe and secured arterial reconstruction procedure for Takayasu's (pulseless) disease has been presented. In order to ensure adequate opening of the arterial lumen at the aortocarotid (-innominate) junction, vascular clamps are partially applied to the aorta at the carotid (-innominate) junction, and open thromboendarterectomy is carried out under direct vision followed by repair with autovein patch plasty. Among 17 patients operated on from November 1966 to February 1973 using this technique, one died of cerebral hemorrhage immediately after the operation and remaining 16 patients took uneventful postoperative course (operative mortality of 5.9 per cent). Patency rate of 100 per cent was observed in the follow-up period, and satisfactory improvement of visual and neurological signs and symptoms followed. The use of prosthetic bypass grafting reported to be successful in arteriosclerotic occlusive lesions is not advisable in Takayasu's disease. |
