Popis: | Summary Surgical therapy is considered for pulmonary hypertension in: 1) high flow left-to-right shunts in a small vascular bed in respect to either obliteration of the shunt or banding the main pulmonary artery; 2) low flow left-to-right shunt with moderately increased pulmonary vascular resistance with complete repair but with a vigorous schema of pre-and postoperative care including chronic tolazoline therapy to remove the vasoconstrictive factor and strict blood gas tension control; 3) balanced systemic-pulmonary vascular systems with an experimental procedure, unilateral pulmonary artery banding; 4) left atrial hypertensive disease with an abnormal pulmonary to atrial pressure gradient with standard operations but exquisite preparation and care; and, 5) acute obliteration of the major portion of the pulmonary arterial tree with accurate diagnosis and medical or surgical embolectomy. |
