Subcutaneous pedicle flaps for reconstruction of the lining of the lip and cheek

Popis: CARCINOMA of the oral region is common in Thailand especially amongst people who chew betel-nut flavoured with slaked lime, tobacco and other spices. Their oral hygiene is poor and they are usually undernourished and exhibit a vitamin B complex deficiency. They seek surgical intervention only when the growth is advanced and unbearable. There is little place for multi-stage techniques of repair because the patients cannot afford long hospitalisation and the bed availability is limited. Surgery must therefore be designed to provide the maximum possible correction in I or at most 2 stages. When resection creates a large through and through defect of the lips and/or cheek, immediate repair is imperative. The purpose of this paper is to describe the use of local skin flaps with subcutaneous pedicles (Barron and Emmett, 1965) turned into the mouth as lining. The skin defect is covered by either a forehead flap, a deltopectoral flap, a cheek rotation flap or a free graft.
