A 10-year-old boy presented to the emergency room with sudden onset severe back pain and headache. He had lower back pain; decreased sensation and progressive weakness of lower limbs; and urinary urgency, frequency and urge incontinence for 4 months. Examination showed fever, tachycardia, neck rigidity, positive Kernig’s sign bilaterally diminished knee reflexes, 70–80% sensory loss to pain, temperature and touch in D12–L5 dermatomes bilaterally.
