Popis: | The incidence of cervical cancer in New Zealand has fallen irregularly since about 1956 in women 35 years and older but has risen since 1959 in those aged 20-34 years. Cytology screening the intensity of which tripled from 1962-1966 cannot be regarded as causal for either event. Mortality in women 35 years and older has fallen since 1959 but has risen for 20-34 year-old women since 1959; neither event can be ascribed to cytology screening. A rising hysterectomy rate has probably contributed a little to the falling incidence and mortality rates in women 35 years or older. The data suggest that cytology screening has not favorably affected incidence and mortality rates for cervical cancer; these have actually risen in women 20-34 years old. Canadian cytology screening studies of comparable intesity to the New Zealand program reported claim that mortality falls are related to intensity of cytological screening. These claims do not seem justified and the significance of "precancers" revealed by cytology and the value of population screening seem doubtful. |
