Foeto-maternal outcome following vacuum extraction: recent experience with 625 consecutive cases

Popis: | Six hundred and twenty five consecutive vacuum extractions in an eight years period is reviewed. The overall success rate was 97 percent and 21.4 percent were done in the first stage of labour. The occurrence of a vesico-vaginal fistula in 0.16% of cases is emphasised as a very serious maternal complications. The corrected perinatal mortality rate was 1.7% and major foetal scalp lesion occurred in 4.8% of cases. The vacuum extractor is presented as a safe instrument for use especially when positional problems are present in the shallow African pelvis. Extra care and experience is required for vacuum extraction in the first stage of labour especially with the extreme perineal muscle laxity in the grandmultipara in order to avoid serious damage to maternal tissues. Careful choice of patients and adherence to the principles of vacuum cup application and traction techniques are emphasised in order to reduce the incidence of foeto-maternal complications. |
