Bell's palsy: prognostic criteria and evaluation of surgical decompression

Popis: | 232 patients with Bell's palsy were examined within two weeks after the onset of the paralysis. 25 patients with presumed bad prognosis (according to clinical and electromyographic criteria) were admitted to a controlled therapeutic trial and randomly allocated to a control or surgical group. 1 patient refused operation, and thus 11 patients were operated on within three weeks after the onset of the paralysis, whereas 14 were not. After a follow-up period of at least a year, no difference was noted between the surgical and non-surgical group. Recovery was poor in most of these cases. 207 patients with presumed good diagnosis recovered completely or nearly so. It was concluded that a prognosis can be made on the basis of our criteria, and that the natural course of Bell's palsy was not influenced by decompression of the facial nerve in the second or third week after the onset of paralysis. |
