A prospective study of plasma-renin activity in normal and toxaemic pregnancy

Popis: | Fifty-eight apparently healthy pregnant women were investigated between the 13th and 27th weeks of pregnancy and then followed up to see whether those who subsequently developed toxaemia were distinguishable from the remainder in terms of plasma-renin activity. Activity was measured before and after 1 week on a low-salt diet and a thiazide diuretic. The mean renin activity was higher in the ten women who later had toxaemia of pregnancy (2816 ng. angiotensin II per 100 ml. per 3 hours) than in the forty-eight women whose pregnancy was uneventful (1795). Eight of the toxaemia group and thirty-five of the remainder satisfactorily completed the week of sodium restriction, and again the mean value for the toxaemia group (6035 ng. angiotensin II per 100 ml. per 3 hours) was higher than that for the normal group (2729). The low-sodium stimulus resulted in good separation of the two groups, so that six of eight toxaemic women but only three of thirty-five unaffected women had renin activity values in excess of 4000 ng. angiotensin II per 100 ml. per 3 hours. It is suggested that the elevated renin activity of pregnancy is one of several aetiological factors in the development of toxaemia of pregnancy, and that those women with more striking elevation in early pregnancy are more prone to develop toxaemia later in pregnancy. |
