Popis: | The number of persons living with chronic conditions such as Chronic Obstructive Pulmonary Disease, Dementia and Stroke is projected to continue to rise in the decades ahead. One emerging approach to reducing the consequent socio-economic burdens is the self-management paradigm whereby a patient is empowered through education, goal setting, measurement and quantification of health metrics, and feedback. This empowerment enables a patient to effectively manage their own long-term conditions. Here we present the current generic data models for educational content provision, setting goals and recording measurements utilised in the KeepWell self-management technology platform. We provide implementation details for each component and consequently identify areas of future work. |
