RECURRENT LARYNGEAL NERVE PALSY AT THYROID AND PARATHYROID SURGERY

Popis: Recurrent laryngeal nerve palsy caused by surgical procedures was retrospectively studied in a series of 1, 149 patients who underwent an operation for thyroid (971) or parathyroid disease (178) in a 5-year period from 1985 through 1989.The nerve palsy or hoarseness had been present from preoperative period in 25 cases, including one case of Graves' disease, 2 of benign adenoma, and 22 of carcinoma. Among 651 patients with benign thyroid disease, 25 developed hoarseness immediately after operation, and 23 out of the 25 recovered. Of 320 patients with malignant thyroid tumor, 58 developed hoarseness immediately after surgery. There were 40 patients undergoing nerve dissection, 11 out of 19 receiving separation of the nerve, 6 of unknown etiology, and one undergoing nerve cut unintentionally. Of the 58 patients 5 (including the unintentional case) underwent an end-to-end anastomosis of the nerve. Hoarseness spontaneously disappeared in 29 out of the 58 patients. In the other 29 patients, representing 9.1% of all malignant tumor patients, hoarseness was everlastingly remained. Recovery of hoarseness was noted in all 5 patients undergoing end-to-end anastomosis.In this paper postoperatively developed hoarseness and its course were examined by diseases and operative procedures.
