Popis: A 51-year-old man presented to the emergency department with plasma glucose of 1,170 mg/dl (65 mmol/L) and in ketoacidosis. He was treated with intravenous (i.v.) insulin and transferred to the intensive care unit. Review of systems revealed he had lost 45 lb within the month before his hospitalization because of a prolonged bout of gastroenteritis. Past medical history included hypertension and hyperlipidemia but no history of diabetes. He was discharged 5 days later on 90 units of insulin glargine daily and 25 units of insulin glulisine three times a day with meals.
