Popis: | Infradiaphragmatic total anomalous pulmonary venous drainage (TAPVD) is a rapidly lethal congenital cardiac defect requiring early surgical intervention for survival. Six patients with this lesion were operated upon, and the last 3 of 5 survived. Clinical and hemodynamic findings include cyanosis, congestive heart failure, unimpressive murmurs, electrocardiographic signs of right axis deviation and right ventricular hypertrophy without right atrial enlargement, a typical radiologic picture, hemodynamic evidence of pulmonary venous obstruction with higher pulmonary than systemic pressures, and characteristic angiocardiograms. Principles of successful management are (I) early recognition and referral, (2) early cardiac catheterization and angiocardiography, (3) early complete surgical correction, and (4) intensive perioperative management. |
