Therapeutic plasma exchange (PE) has been established as an important adjunct in the treatment of some autoimmune diseases. Behcet's disease has the features of autoimmune disease and multiple reports on the use of PE in small groups of patients have appeared. A systematic study of the possible benefit of combined PE and immunosuppression is still lacking. Serial PE will dramatically lower levels of autoantibodies as well as circulating immune. complexes for a short period, thereby possibly initiating a proliferative response of cell clones which produce the autoantibody. In this phase such clones would be extraordinarily vulnerable to cytoreductive therapy as effected by Cyclophosphamide (Cy). A pulse approach would at the same time limit risks of unwanted side effects. A draft protocol for an international multicenter study of pulse Cy versus serial PE followed immediately by pulse Cy in severe Behcet's disease disease is presented to be evaluated by interested parties such as the International Study Croup for Behcet's disease. |
