Nonmyeloablative Allogeneic Stem Cell Transplantation: Towards Improving Efficacy of Immunotherapy Instead of Myeloablative Chemoradiotherapy

Popis: Myeloablative conditioning of patients undergoing allogeneic blood or marrow transplantation (alloBMT) to treat malignant hematologic disorders is generally considered a mandatory first step although it is associated with severe immediate and late complications. Immune-mediated graft versus leukemia (GVL) effects constitute the major benefit of alloBMT [1]; therefore we focused our attention on the possible use of a non-myeloablative conditioning protocol prior to transplantation of allogeneic blood stem cells mobilized by G-CSF (Neupogen). The aim was to establish host versus graft tolerance which allows engraftment of donor immunohematopoietic cells which should induce GVL effects and displace residual malignant or genetically abnormal host cells. We summarize both our previously published animal data supporting allogeneic non-myeloablative stem cell transplantation (alloNST) and preliminary clinical data that support the feasibility of alloNST as a putative better and safer replacement for alloBMT with prior lethal doses of chemoradiotherapy as practiced today.
