CLINICOPATHOLOGICAL ANALYSIS OF se GASTRIC CANCER WITHOUT LYMPH NODE METASTASIS

Popis: The results of excision for se gastric cancers largely depend on two factors, peritoneal dissemination and lymph node metastasis. Pre-or intra-operative information related to the lymph node involvement may facilitate a more effective, i.e., more biased management against the possible peritoneal dissemination which often leads to a fatal outcome. Clinicopathological features of 24 patients without (Group A) and 133 patients with microscopic lymph node involvement (Group B) having undergone a gastric resection combined with ≥R2 node dissection for a se cancer were retrospectively analyzed. There was a wide discrepancy between the pre-/intra-operative (macroscopic) and postoperative (microscopic) judgement on node involvement. No noticeable difference was found between Group A and B except that a type 3 tumor and peritoneal dissemination were more frequent in the Group B patients with positive lymph nodes. To lessen the degree of node dissection, thereby more concentrating on the management of peritoneal dissemination, intraoperative frozen section histology for node metastasis seemed mandatory.
