Popis: | Introduction: First Aspiration Pass Technique (ADAPT) was shown to be effective, safe and cost effective. However, around third of patients treated with aspiration technique require rescue therapy. Objective: report the rate of aspiration success (AS), investigate the predictors of aspiration success and the effect of aspiration success on the functional and hemorrhagic outcome. Methods: We interrogated our prospectively collected database which included all patients who presented with large vessel occlusion between 1/2013 and 12/2017 and were treated with ADAPT technique. Baseline characteristics, procedural and outcome variables were collected. Functional outcome was assessed using modified rankin scale (mRS). Hemorrhagic transformation was classified based on ECASS radiological criteria. Logistic regression was used to determine predictors of AS and investigate the effect of AS on outcomes. Results: A total of 510 patients were included in analysis. 333 patients (65%) achieved successful recanalization using only aspiration technique. On multivariate analysis, pretreatment tPA and ICA occlusion were associated with AS. mRS 0-2 was achieved in higher proportion of patients in AS group (42 % vs. 31%; p Conclusion: AS was associated with better functional outcome and lower mortality rate. Both IV tPA and ICA occlusion were predictors of aspiration success. |
