Robotic surgical management of sigmoid colon cancer in pregnancy: A case series

Popis: Background Colorectal cancer (CRC) during pregnancy is uncommon and its management can be challenging. Maternal and fetal factors affect treatment options for CRC during pregnancy. In this case series of three pregnant patients with CRC we focus on adjustments made to address technical challenges. Body Three patients were diagnosed with sigmoid colon cancer, at 16, 24 and 28-weeks gestation, respectively. Each patient underwent robotic-assisted colon resection. Uterine size was different between the patients. Correspondingly, different instruments were needed in each patient. A robotic grasping retractor and a non-traumatic grasper through an assist port was used to retract the 16-week uterus. The large and boggy uterus in the two latter patients impacted exposure. Due to the size of the uterus and the risk of injury, a low pfannenstiel incision or peri-umbilical midline laparotomy were used to place a hand-assist port. This allowed manual atraumatic retraction of the uterus. Conclusion This series provides surgical approaches for pregnant patients with colorectal cancer. We have identified technical adjustments to consider when performing a robotic colon resection in pregnant patients depending on gestational age.
