Coronary and cerebral ischemia: surgery in one or two stages

Popis: At the Cardiac Surgery Dept. of the University of Bologna, 45 patients from 41 to 72 years have been treated for coronary and carotid atherosclerotic pathology. Among these patients, 31 have been operated in one stage (group A) and 14 in two stages (group B); in this late case the most serious damaged area was surgically approached primarily. The hospital mortality rate was 3.2% (1/31 pts.) in group A and 27.4% (5/14 pts.) in group B. This was due to myocardium infarction in group A and to cardiac or cerebral causes in group B. The average follow up was 43 months (3 min. – 120 max.): all patients were checked again both clinically and by carotid echo-Doppler examination. The late mortality rate was 6.4% (2/31 pts.) in group A due to probable myocardium infarction. In conclusion we obtained that the best approach in patients with coronary and carotid atherosclerosis lesions would be simoultaneous surgical treatment.
