An assessment of our technique with frozen section preparation and interpretation of cervical cone specimens is presented. It has been possible to obtain a satisfactory evaluation of the lesion present in a large majority of the cases. Among 166 patients subjected to conization of the cervix for evaluation of atypical cells observed on cervical and vaginal smears, frozen sections of the tissue properly were interpreted as benign conditions in 71 cases, carcinoma in situ was recognized in 48 (including 1 in which microinvasive foci were also noted in the frozen tissues), and invasive Stage I cervical carcinoma was demonstrated in 7. In 3 additional cases in which only carcinoma in situ or microinvasive foci were seen on the frozen sections, foci of invasion were also found in the permanent sections and, in one case of dysplasia and cervicitis, the frozen sections were interpreted as carcinoma in situ. These frozen sections yielded information with which appropriate definitive therapy could be instituted immediately in 52 of 80 patients with carcinoma in situ. The necessity of correlating the tissue sections with the cytologic smears and of ascertaining the tissue source of atypical cells observed on the smears is stressed. |
