An 11-year-old boy undergoes transsphenoidal resection of a craniopharyngioma and develops post-operative dysnatremia that follows the triphasic response. This case will discuss the challenges and general principles of management for patients who develop post-operative diabetes insipidus (DI), followed by hyponatremia due to SIADH, and end with permanent DI. Challenges specific to the pediatric patient and post-operative complications are discussed as well. Finally, recommendations for as-needed dosing of oral desmopressin are provided to help facilitate management of potentially transient DI. |
