A scoping review investigating the impact of genetic counsellor involvement in genetic and genomic test order review

Popis: Objective: The aim of this scoping review is to provide an overview of published, peer-reviewed, and other literature on genetic counsellor involvement in genetic test order review and to identify gaps in our current knowledge. The objective is to determine the extent of the impact of having genetic counsellors involved in the test order review process. Introduction: The rise in the availability and complexity of genetic technologies, paired with more genetic tests being ordered by health care professionals with limited genetics and genomics training, has resulted in an increase in the number of inappropriately ordered tests. Genetic counsellors are ideally suited to assess the appropriateness of a genetic test in light of the patient’s medical and family history. Employment of genetic counsellors in the laboratory has been implemented widely as a solution to the issues surrounding clinical test misorders. The current scoping review will focus on the impact of having genetic counsellors involved in the test order review process. Inclusion criteria: The review will consider studies that investigated the impact (ex. economic, clinical etc.) of having genetic counsellors be involved in reviewing genetic test orders for appropriateness. Articles that evaluate utilization management programs where genetic counsellors were not involved will be excluded. As this is a scoping review intended to capture the breadth of knowledge, no restrictions will be placed on study design. Methods: This scoping review will follow the format outlined by the Preferred Reporting Items for Systematic Reviews and Meta-analyses extension for Scoping Reviews (PRISMA_Scr) guidelines. The databases, which include MEDLINE (via OVID), EMBASE (via OVID), CINHAL (via EBSCOHost), EBM reviews (Cochrane) and Web of Science Core Collection, will be searched using a prescribed search strategy created with the assistance of a research librarian. Additionally, we will search the grey literature and guidelines statements from key professional organizations. Articles that meet the inclusion criteria will be included and analysed as they relate to the research question. The identified sources will initially be screened (titles and abstracts) by two independent reviewers. Sources that are duplicates or do not conform to the inclusion criteria will be excluded at the initial stage. The second screening will be conducted using full-text studies by the same two independent reviewers to analyse the inclusion of studies. A predefined charting form will be constructed to extract relevant data from the included sources. Results will be presented in diagrams, tables, and descriptive formats in alignment with the research questions. The results will serve to highlight implications for further research or possible practice implications.
