A comparison of MRI findings in cases with progressive massive fibrosis and lung cancer

Popis: Objective: The objective of this study is to evaluate the effectiveness of MRI and Diffusion-Weighted Imaging (DWI) in the differential diagnosis of PMF and lung carcinoma. Materials and Method: From September 2017 to September 2019, we recruited consecutive patients presenting with suspicious masses on radiological exams in a single large referral center. A total of 40 cases who were clinically and radiological diagnosed with pneumoconiosis according to International Labour Organization Classification and 11 cases who were diagnosed with lung cancer based on clinical, radiological and pathological findings were included in the study. Results: Total of 27 cases (40.30%) in the PMF group and 2 (15.38%) cases in the lung cancer group presented with bilateral lesions. The incidence of heterogeneity was higher in patients with cancer (p=0,000). On T2 weighted (T2W) images, the number of isointense lesions (48, 71.6%) was higher in the PMF group, whereas the number of hyperintense lesions (8, 61.5%) was higher in the lung cancer group (p=0,000). The Apparent Diffusion Coefficient (ADC) values were lower in PMF cases (p=0.142). Discussion: We detected a restricted diffusion pattern (hyperintense appearance) in malignant lesions on DWI. The results of our study also revealed that isointense or hypointense signal patterns were more common in PMF lesions both onT2W and DWI images.
