Popis: | The ideal setting to diagnose, study and treat ‘dermatology in gynaecology’ is the vulval clinic, staffed bydermatologists, mainly women, and often held jointly with gynaecologists. The disorders seen in the majority of patients fall into three groups, vulval dermatoses (lichen sclerosus, lichen planus, eczema, psoriasis and blistering diseases), pre-malignancies (vulval intra-epithelial neoplasia and lichen sclerosus) and pain syndromes (vulvodynia and vulvar vestibulitis). Neither immediate surgical intervention nor urgent infection investigation is required, but patients should be offered the treatments that are available (this may include very potent topical steroids, safe to use with supervision). Although there may not be a ‘cure’ for these problems, making a prompt diagnosis (often involving a simple vulval skin biopsy performed during the first outpatient visit) and instituting treatment to alleviate symptoms is important, as is ensuring adequate follow-up to detect associated problems and/or malignancy. Vulval clinics are now widespread in the UK and prove popular with patients. They help women come to terms with conditions that may be a life-long problem. |
