A RESECTED CASE OF GIANT CAVERNOUS HEMANGIOMA OF THE LIVER

Popis: A 53-year-old female complaining of upper abdominal pain and weight loss was diagnosed as having liver cavernous hemangioma (LCH) of the giant type, and a segmentectomy of the right lobe of the liver was carried out. Nothing was done to the small hemangiomatous lesions located in the left hepatic lobe during the surgery. The resected liver weighed 218g and contained an 8×6 cm tumor, whose histology revealed a typical cavernous hemangioma. Her postoperative course was uneventful. Since 1980, 139 adult surgical cases of LCH including our own have been reported in the Japanese literature. The male-to-female ratio was 1:1.5, the incidence of LCH was higher in the 40's and 50's, and there was no significant difference in average age between the sexes. Among the symptoms of LCH, abdominal pain and a palpable abdominal mass were each noted in 34 percent of the patients. LCH with a maximum diameter of more than 4cm was noted in 104 of 112 cases (93%). Among the selected operative procedures, a hepatectomy that was more aggressive than segmentectomy was carried out in 85 out of 139 cases (61%). If residual small hemangiomas are also present, longterm follow-up should be done.
