Surgical Indications and Postoperative Complications in Patients over 70 Years Old with Lung Cancer

Popis: | One hundred and fifty seven thoracotomies were performed in patients with lung cancer over the age of 70 years from July 1976 to June 1987. This number corresponds to 26.2% of all thoracotomies for lung cancer. Patients ranged in age from 70 to 82 years with a mean of 73.8 years. There were 125 men and 32 women. Our surgical indications were as follows : 1) expected postoperative percent vital capacity over 40%. 2) PaO2 more than 60 mmHg and PaCO2 lower than 50 mmHg in preoperative arterial blood gas analysis. 3) Unilateral pulmonary occlusion test should be applied in patients with borderline pulmonary function. Those with a mean pulmonary arterial pressure over 2E mmHg are not expected to tolerate pneumonectomy. Furthermore, patients with heart failure and those who had had a myocardial infarction within 3 months were excluded from thoracotomy.Postoperative cardiovascular complications were seen in 15%, respiratory problems in 34%, psychiatric disturbances in 10% and miscellaneous in 13%. A high incidence of retained secretions and atelectasis was observed in patients with FEV1.0% values below 70, MVV lower than 1.5L/S or obstructive index over 2.5. The operative mortality was 5.1%. Five year survival rates 44% for stage I cancer, 24% for stage II, 5% for stage III and 0% for stage IV. |
