Management of bleeding complications after bariatric surgery

Popis: RESULTS: Overall, 1532 patients underwent either LRYGB or LSG. Twenty-nine patients (2%) had a bleeding complication. All of the bleeding complications occurred after LRYGB; one patient underwent a concurrent Nissen fundoplication takedown. Twenty patients (62%) with bleeding complications required a transfusion, and 8 (28%) were taken back to the OR for re-exploration during their index hospitalization. Seven patients were readmitted for bleeding complications at a median of 9 days (range 2e15) postoperative. Location of bleeds included the gastrojejunostomy (n1⁄44), jejunojejunostomy (n1⁄42), port sites (n1⁄42), and staple line (n1⁄45). The site of bleeding was not determined in 2 patients. Thirteen (45%) patients were transfused for acute blood loss anemia. In 2004, we revised our anticoagulant from enoxaparin to heparin. This change resulted in decreased rates of bleeding complications, from 6% (14/239) to 1% (14/1293). There were no 30-day mortalities.
