Searching for an objective evaluation of patients with cerebral palsy after stem cells treatment. Is functional magnetic resonance imaging a solution?

Popis: | Background & Aim Stem cell based therapy (SCT) is an alternative for diseases that, until some years ago, were considered incurable, such as cerebral palsy (CP). Efficacy of SCT has been proved subjectively only through the improvement of quality of life, and enhancement of behavior of patients with CP. Functional magnetic resonance imaging (fMRI) may be used as a tool that can measure objectively morphological changes that occur in the brain of patients with these interventions. Methods, Results & Conclusion We presented three pediatric patients with severe neurological deficit as consequence of hypoxic-ischemic encephalopathy. They received intrathecal autologous bone marrow (BM) derived total nucleated cells after BM stimulation with granulocyte colony-stimulating factor, demonstrating improvement of their neurological functions. A basal fMRI was obtained and compared with a second fMRI at least 6 months after treatment, demonstrating increasing of the fractional anisotropy (as shown in table 1), which correlates with their clinical improvement. It has been demonstrated that hematoprogenitor stem cells are capable of enhancing neurogenesis and angiogenesis in affected hypoxic-ischemic brain tissue. Functional MRI is becoming a powerful tool to evaluate objectively changes occurring in central nervous system, specifically in the white matter tracts. Randomized studies are needed to support this possibility. |
