Osteoporosis, PTH elevada y calcio iónico inadecuadamente 'normal'

Popis: The measurements of calciotropic hormones and serum calcium routinely in evaluation of patients for osteoporosis, carries to identify normocalcemic subjets with elevated parathyroid hormone. We describe a case of a 53 year-old woman with osteoporosis who has high level of parathyroid hormone with inapropiately normal levels of ionized calcium. In the nuclear magnetic resonance of the neck appears a cystic tumor compatible with parathyroid adenoma. Cervicotomy was done due to the severe bone mass decrease and the discovery in the MRI. Pathological description of chirurgic piece confirms the diagnosis of normocalcemic primary hyperparathyroidism. We revise the possible ethiology and discuss this unusual way of presentation of primary hyperparathyroidism
