Acute Thrombosis of Vascular Access for Hemodialysis: Incidence and Management in France

Popis: Objectives: The acute thrombosis of a vascular access (TVA) is a serious complication of hemodialysis. The real incidence is not known and the management, which could influence the results, is variable according to the centers. The objective of this national survey was to evaluate the real incidence of TVA in France, and to specify the methods of their management. Materials and Methods: Between Mars and April 2013, a national nephrologist survey was conducted in the 269 centers of dialysis of Metropolitan France; 266 took participated. The data collected were the number of dialyzed patients, the percentage of permanent catheters, the incidence of TVA, the specialty of the consultant treating the thrombosis (vascular surgeon, interventional radiologist or urologist), and the time until the treatment. The data were compared between the 22 departments of more than 1 million inhabitants (D>1M) and the 74 departments of less than 1 million inhabitants (D 1M (V: 49%, R: 51%), even higher in Ile-de-France (V: 12%, R: 88%). In the remaining centers, 2% referred the patient to an urologist whereas 13%chose the specialist according to the type of foreseeable procedure. The time until treatment was 0.05). Conclusion: The incidence of TVA is 8 per 100 patients/ year in France. In 91% of the centers the treatment is proposed within 48h, which corresponds to the European recommendations. There exists a strong regional disparity in the choice of the specialty of the consultant.
