JEJUNAL INTERPOSITION HEPATICODUODENOSTOMY AS RECONSTRUCTIVE SURGERY FOR BENIGN BILIARY DISEASE

Popis: We evaluated the clinical results of jejunal interposition hepaticoduodenostomy (7 cases) for congenital choledochal dilatation or benign biliary stricture from the standpoint of bile excretion using biliary scintigraphy and gastroscintigraphy performed simultaneously. As compared with Roux-en-Y hepaticojejunostomy (7 cases), jejunal interposition hepaticoduodenostomy showed rapid excretion of bile into the upper intestinal area, and no postcibal asynchronism was observed. However, this reconstructive procedure required 15-20 cm of jejunal segment to show physiological postpranidal bile excretion. One case which was reconstructed with jejunal interposition hepaticoduodenostomy with 40 cm of jejunal segment showed remarkable congestion of bile within the interposed segment, and the degree of postcibal asynchronism was similar to that in Roux-en-Y reconstruction. Thus, length of the jejunal segment is one of the important factors in jejunal interposition hepaticoduodenostomy to minimize postcibal asynchronism.
