Monitoring Vitamin D Concentrations in Patients with Osteoporosis: A Clinical Perspective

Popis: The role of vitamin D has become a highly discussed topic in the media and medical literature, as it has been postulated to have important and beneficial effects in multiple systems of the body. In this issue of Pharmacotherapy, Drs. Jennifer Isenor and Mary Ensom discuss the role of therapeutic drug monitoring of vitamin D for fracture prevention. The authors use a nine-step decision-making algorithm to review data regarding monitoring vitamin D concentrations. Their article deduces the following: evidence for fracture prevention with vitamin D supplementation is inconclusive, current assays for measuring vitamin D concentrations yield varying results, the existing primary literature shows no clear relationship between vitamin D concentrations and fracture risk, target vitamin D concentrations are not well established, and it is unlikely that results of a vitamin D assay will make a significant difference in the clinical decision-making process. Based on this reasoning, they conclude that there is insufficient evidence at this time to recommend routine therapeutic drug monitoring of vitamin D. It is our goal to provide a clinical perspective regarding monitoring vitamin D concentrations in patients with osteoporosis. To illustrate this perspective, we present a case study of an elderly woman who received therapeutic drug monitoring of vitamin D levels while taking very high doses of vitamin D supplementation, which was necessary in her case to reach a sufficient vitamin D concentration.
