Ninety-three resected cases of primary lung cancer were reviewed in terms of postoperative recurrence. The recurrence occurred in 29 cases (31.2%). When the recurrence rate was calculated by stage and histologic type, the rates were 17.5%, 80.0%, 33.3%, and 80.0% in stage I, II, IIIA, and IV, respectively; and histologically, 31.7%, in adenocarcinomas, 19.0% in squamous cell carcinomas, and 66.7% in adenosquamous carcinomas. Regarding recurrence mode, vascular recurrence such as lung, bone or brain recurrence was the most frequent (82.8%). Most of stage I and II cases showed vascular recurrence more than one year after operation whereas majority of stage III and IV cases showed lymphatic and local recurrences within six months after lymphatic and local recurrences within six months after operation. Histologicaly, all recurrences in squamous cell carcinomas appeared more than one year after operation while many of recurrences in adenocarcinomas appeared within one year. About 40% of recurrent cases received only symptomatic treasments and post-recurrence survival period was 4.3 month on an average. Chemotherapy and irradiation were effective on some cases of bone or brain recurrence.This study suggests that primary lung cancers often recur including those in earlier stage, and thus strict follow-up for early detection and treatment of recurrence is inevitable. |
