“Pilon” or “plafond” fracture pattern is defined by the intra-articular involvement of the distal tibia with metaphyseal extension. It only occurs in a small percentage of tibia and lower extremity injuries. One third of pilon fractures stem from high-energy mechanisms of injury and they are often associated with concomitant polytrauma. Historically, treatment involving early acute open reduction and internal fixation (ORIF) led to clinical outcomes with high complication rates. The implementation of a delayed and staged surgical treatment protocol, along with the improvements in imaging, implant technology, and surgical techniques, has allowed us to decrease the complication rates. Some recent authors have suggested that early definitive ORIF can have comparable results with staged protocols. |
