Popis: The cause of chronic urticaria is often elusive and may be multifactorial. Routine laboratory investigations, other than a blood count and erythrocyte sedimentation rate (ESR), are usually unhelpful. Non-specific precipitants, such as food colours and additives and non-steroidal anti-inflammatory drugs, should be avoided if possible. Heat, exercise, stress and alcohol consumption should be minimized. H, receptor blocking antihistamines are the mainstay of drug therapy. A non-sedating antihistamine may usefully be combined with a sedating antihistamine at night if sleep is disturbed. The addition of an H, receptor antagonist may be beneficial. Oral terbutaline and nifedipine can sometimes be effective when taken with antihistamines, probably by inhibiting degranulation. Treatment with antifibrinolytic agents and attenuated androgens should be reserved for severe refractory cases that have not responded to antihistamines.
