Le NT-proBNP dans l’insuffisance cardiaque des sujets noirs africains : quels seuils faut-il retenir en ambulatoire ?

Popis: Summary European and American recommendations since 2007 have validated the use of NT-proBNP in the diagnosis of heart failure. Few studies have been conducted on these markers in African patients after an episode of heart failure. The objective of the work was to evaluate the diagnostic value of NT-proBNP (VIDAS ® NT-proBNP) in 51 black African adults with dilated cardiomyopathy, outpatients, from threshold values of international recommendations. These values were compared to clinical diagnosis of heart failure according to clinical criteria of Framingham. For the diagnosis of heart failure, NT-proBNP has a positive predictive value (PPV) of 94.3% and negative predictive value (NPV) of 62.5%. For the diagnosis of exclusion, at the threshold of 300 ng/L, the sensitivity and specificity of NT-proBNP were, respectively, 92.5% and 50%. The area under the ROC curve is 0.91 CI 95% with an optimal threshold of 532 pg/mL, for which we obtained a PPV of 94.6% and NPV of 71.4%. Finally, NT-proBNP can be used in the African patients who have had an episode of cardiac decompensation with the same threshold values than those recommended internationally.
