A CASE OF LYMPHOEPITHELIAL CYST IN THE HEAD OF THE PANCREAS

Popis: A case of lymphoepithelial cyst (LEC) in the head of the pancreas is reported. A 73-year-old man complaining of general fatigue and anorexia was admitted to the hospital, since he was found to have a cystic lesion in the head of the pancreas by an abdominal ultrasonography. He had no remarkable previous histories such as abdominal trauma and had not drunk alcohol heavily. On blood chemistry, no abnormal findings were noted, though CA 19-9, Span-1, and Ca-50 increased to 163 U/ml, 49 U/ml, and 58 U/ml, respectively. Abdominal CT and MRI demonstrated a well-circumscribed cyst measuring approximately 5.0cm in size. Endoscopic retrograde cholangio-pancreatography (ERCP) only revealed oppressed main pancreatic duct toward the head side. Upon laparotomy, an elastic-soft mass in the head to the uncus of the pancreas was present and a pylorus-preserving pancreatoduodenectomy (PPPD) was performed. Histopathologically, the cyst wall was lined by mature, keratinizing squamous epithelium and surrounded by lymphoid tissue with lymphoid folli-cles. It was diagnosed as LEC of the pancreas. LEC of the Pancreas is a rare lesion and only about 30 cases have been reported in the world.
