Written discharge instructions in the hospital setting are frequently underutilized or written at literacy levels exceeding the general population’s ability to read and understand. This limited comprehension can have negative health consequences. Prior studies have shown improvement in outcomes when other forms of discharge instructions are utilized, including video. Implement a quality improvement project to initiate video discharge instructions in the pediatric emergency department. The aim is to determine the effectiveness of video discharge instructions in improving caregiver knowledge and satisfaction. A pilot study was conducted at an academic level I pediatric trauma center. Participants included English speaking caregivers of patients aged 0-17 years, presenting with fever. The primary outcome was knowledge about fever, treatment plan, and follow-up assessed from a questionnaire. The secondary outcome measured satisfaction using a Likert scale. Twenty caregivers watched the novel discharge video and participated in the surveys. Two participants chose to complete the surveys without watching the video. The demographic data revealed 85% of the caregivers were female, 65% were white, and 80% were non-Hispanic. Sixty percent of participants completed greater than high school education while 40% had less than 12 years of education. There was a statistically significant difference in results of the questionnaire from participants who completed greater than 12 years of education compared with participants who completed less (P = 0.028). There were no statically significant knowledge gains when comparing those who viewed the video versus those who did not irregardless of education level. All participants who responded to the survey rated that they were satisfied or highly satisfied with the video. This study demonstrated lower levels of education resulted in lower scores on a fever quiz, despite watching an instructional video. The video was well received among the study cohort and highly rated by caregivers. Thorough communication and discharge education are an integral part of patient care delivery and good patient outcomes. Based on the positive feedback from caregivers, this intervention should continue to be a routine part of discharge planning.
