Comparative study of bronchial provocation tests using methacholine or mannitol in bronchial asthma

Popis: Introduction: The bronchial provocation tests (BPT) are useful in confirming the diagnosis of bronchial asthma (BA). BPT are usually performed with methacholine (Me) and mannitol (Ma) is recently being used as well. Aims: Analyzing the profitability of both BPT in the diagnosis of suspected BA. Methods: We included 108 patients suspected BA without pretreatment . BPT were performed randomly and on different days. Clinical, functional and (FeNO) variables were analyzed. For the correlation between BPT, researchers performed Kappa index. Sensitivity (S), Specificity (Sp) positive and negative predictive value of Me and Ma and the confidence interval (CI) 95% were analyzed. Results: The mean age of our patients was 36, 99 years. After making the BPT BA was excluded in 52, 8% of individuals referred for suspected BA. Four groups of patients were obtained: Me-/Ma-; Me-/Ma+; Me+/Ma- y Me+/Ma+. The BPT with Ma was positive in 23, 1%, and 76, 9% were negative. In 24 patients with positive Ma BA (100% with asthma effort) was diagnosed. According to the Kappa concordance index for Ma was 0, 43 (p Conclusions: Both BPT have similar values of S and Sp in the diagnosis of BA. Meaning the low concordance between the two tests in patients with compatible symptoms or negative Ma we should make an additional BPT to confirm the diagnosis.
