SUMMARY The basic tenet in stabilizing lax ligamentous tissue is to provide passive stability reinforced by dynamic motor power to continually protect these structures against abnormal motion. The latter is accomplished on the medial side through the pes anserinus muscle group in pes anserinus transfer, by the semimembranosus complex in posterior medial capsular reinforcement, and by the vastus medialis and quadriceps in the patellar tendon-split transfer. Laterally the biceps femoris, popliteus, and lateral portion of the gastrocnemius provide the muscle tightening mechanism to the reconstructed ligaments. Restoration of joint motion is done by active exercises to allow the muscles to stretch and accommodate to their stabilizing function as their strength is regained. |
