Popis: Schwartz and colleagues 1 recently reported, in the Images for Surgeons section, an interesting case of leftsided appendicitis in a 38-year-old man. We agree with the authors about the ease and practicality of laparoscopy in dealing with this clinical problem. The author’s recognition that this entity occurs in patients with intestinal malrotation is correct, but a diagnosis of malrotation warrants special consideration. We would refer the authors to a review by Kapfer and Rappold. 2 Although management of the asymptomatic patient with malrotation is controversial, 3 a discussion between the surgeon and patient about the risk of midgut volvulus is salient. Laparoscopic Ladd’s procedures have been performed successfully with minimal morbidity. 4 Because the risk of volvulus cannot be predicted, it may be prudent for certain intraoperative steps to be taken to minimize future risk of complications. 4
